A company is looking for a Crisis Care Manager.
Key Responsibilities
- Provide emotional support and care coordination for families of behaviorally and medically complex children
- Assess case referrals, identify resource needs, and monitor case progress for closure
- Connect families with community resources and support groups to aid in recovery goals
Required Qualifications
- Master's degree in Social Work (MSW) required
- Bilingual in English/Spanish preferred
- 2+ years of case management or relevant experience
- Experience in child & adolescent case management and acute behavioral health needs
- Strong knowledge of community resources
