A company is looking for a Lead Case Manager, Bilingual (Spanish).
Key Responsibilities
- Serve as the primary contact for members and their support systems, coordinating care with a multidisciplinary team
- Assess member needs and develop comprehensive Care Management Plans to address gaps in treatment and services
- Implement Care Plan services, advocate for members, and connect them to necessary social services and supports
Required Qualifications
- Fluency in Spanish
- High School Diploma or GED required; Bachelor's degree preferred in a relevant field
- 2+ years of experience in care management or mental health services with Medi-Cal members
- Current California driver's license with an acceptable driving record
- Comfortable working with diverse populations in various environments
